Sample letter changing return date from maternity leave

22rd May 2014

Dear Ms Hall,

As you know, before my maternity leave I told you that I would return after six months, on 19th September.

However I have decided that I will take all my 52 weeks maternity leave and my return date will now be 19th March 2015

Yours sincerely,

Sian Moore
